Community Navigator

Granite Pathways has contracted with the Division for Children, Youth, and Families (DCYF) to provide Community Navigator services throughout New Hampshire. The Community Navigator Program is designed to outreach families and reporters with the goal of enhancing their knowledge of the appropriate community supports and resources available in their community. The overall goal is to help educate reporters and provide them with a helpful skillset to work directly with the families; to help shift their role from just reporting concerns to actively supporting families in need. This program is also designed to help families navigate and engage with the available community support and resources designed to assist families in need.

Community Navigator services are financed under a contract with the State of New Hampshire, Department of Health and Human Services, with funds provided in part by the State of New Hampshire and/or such other funding sources as were available or required, e.g., the United States Department of Health and Human Services.
